Where to Begin
If you would like your child to become a patient, take these steps to become a new patient of Dr. O'Hara:
- Read the Frequently Asked Questions (FAQ),
- Fill out the Pediatric Initial Contact Form (either Printable PDF or Microsoft Word),
- Sign the Pediatric Patient Consent Form,
- Sign the HIPPA Notice of Privacy Practices form,
- Sign the Practice Policies Form,
- Fill out the Pediatric Patient Questionnaire.
Please send all of the above forms to us with a $150 deposit (click here for our mailing address).
Once we receive this, our office manager will contact you by telephone to schedule your initial consultation.
Or if you prefer, you may contact the office for all necessary forms to be mailed or faxed to you and proceed from there. When we receive your registration and deposit, we will then send you our questionnaire via mail.
We will then contact you to schedule an appointment.
