Adult Healing and Wellness - Where to Begin
Please follow the steps below if you wish to become a patient of our practice:
- Download and complete the Adult Initial Contact Form (either Printable PDF or Microsoft Word)
- Download and sign HIPPA Notice of Privacy Practices Form
- Download and sign the Practice Policies Form
- Download and complete the Adult Patient Questionnaire
- Send all of the above back to use with a $150 deposit (this will be applied to the cost of your initial visit). Once we receive the information and deposit, our office manager, will contact you by telephone to schedule an initial appointment. In order for the clinicians to adequately review and prepare for each visit, under most circumstances, new patient appointments will be scheduled at least 2-3 weeks after all of the above is received by our office.
Please note: Dr. Szakacs does not participate in any health insurance. Therefore, it is up to you to contact your insurance company in order to determine what reimbursements, if any, can be expected before committing to our program. We will, however, provide you with all documentation required by your insurer.
